He was appointed as a Probationary Flight Sub-Lieutenant (Temp.) RNAS (With seniority dated 17/11/1916). He served in Crystal Palace for Training. He served in Vendome, Freiston and Dover for training. He was appointed as a Flight Sub-Lieutenant RNAS 1917. He served in #6 (N) Squadron, he scored his first victory with a Sopwith Camel on 27 July 1917. He served in #1 (Naval) Squadron (later #201 Squadron) 1917. (Transferred to RAF 1918). He served in #201 RAF Squadron 1918. He was appointed as a Captain (Temp.) RAF 1918. #70 Squadron for Special Service 1918. Before this squadron was re-equipped with Camels, Forman scored his second victory with a Sopwith Triplane. He achieved six more victories with the Camel before joining 70 Squadron on special assignment as a flight commander. After scoring his final victory in August, he was shot down and captured on the morning of 4 September 1918. He was shot down and made prisoner-of-war on 08/09/1918. He was repatriated 10/12/1918. He transferred to the RCAF in 1920s. He served in World War Two as a Flight Lieutenant RCAF. |
