He was the son of William Joseph and Mary Formby, Powell River BC. He was buried in St. Andrews Anglican Church Cemetery Courtenay BC.

 

Military Service
He was appointed as a Wireless Telegraph Operator (4th Class) RNCVR (With seniority dated 01/03/1916). (He died of illness 19/07/1917).
